This sales contract is made between Lisa Duvall, hereinafter termed BREEDER , address *********** Slippery Rock, Pa. 16057, and:

The BREEDER agrees to sell, and hereby does sell to BUYER a Standard
Poodle, male/female, whelped _____ ______

General:
Buyer agrees that this dog will never be left unattended while tethered to a chain or rope. ________________________
Buyer agrees to annual veterinary check-ups. _____________________
Buyer agrees to provide Breeder with photos of this dog at approximately 6 and 18 months of age. ____________________________________
Buyer agrees to notify Breeder immediately of any and all problems relating to the dogs physical health, disposition, personality, and social/family adjustments. ____________________________
Buyer agrees if for any reason in the future Buyer finds it necessary to not keep the animal, Breeder reserves the right to have the dog returned to them or assist in finding a suitable home. At no time is this dog to be disposed of without Breeders permission or to be placed in an animal shelter of any kind. __________________

Buyer agrees this dog shall not be bred and is sold on a limited AKC registration.

Buyer agrees to have this dog spayed/neutered prior to dogs first birthday and will provide proof to Breeder this has been done. _________________________
Guarantee:
Breeder guarantees this puppy will become a normal pet quality dog.
Breeder guarantees this puppy has been vaccinated against distemper/parvo and checked by a professional vet.
Breeder guarantees this animal for 2 years from genetic health issues including only the following:
* severe hip dysplasia ++
* PRA ( Progressive Retinal atrophy)
* Legg-Calve-Perthes
* vWD ( Von Willebrands disease)
Buyer agrees this dog will be diagnosed by two ,unrelated in practice, vets and by Breeders vet at Buyers expense prior to upholding this guarantee.
If at such time it is determined by these 3 vets it is one of these four genetic
diseases Breeder has 2 years to return full purchase price ($1000) or puppy of equal quality to Buyer, from date of last diagnosis from Breeders Vet.
Buyer agrees to take dog to their vet within 2 days of purchase, Breeder will return Buyers purchase price if at such time a defect or health issue had been found and the dog will be returned to Breeder. If Buyer decided to keep this dog no refund will be given now or in the future for any reason, the full guarantee will be void.
Breeder guarantees dog free from parvo/distemper for 48 hours from purchase date. At Buyers expense an ELISA test must be preformed within 48 hours if claiming either of these viruses.
Breeder only guarantees against issues stated under the Guarantee clause, any other problems are not covered and are the responsibility of the Buyer.
